Isn’t this an indication that the solutions that were taken in previous times were not correct solutions? Or was the problem in the previous diagnosis of the problem?

Have you noticed that most of the results of accident investigations indicate that the main cause of that accident are people and that every investigation of an accident results in the application of a penalty to a person? However, the same problem is repeated in reality, and the losses may be much greater in the coming times.

Studies indicate that 85-95% of the causes of disparity in the performance of any organization are due to the “system”, while only 5-15% of those reasons are due to humans. So why do the results of the investigations usually indicate the opposite, and almost all of them put the blame on people for any problem, malfunction or accident that occurs?

Root cause analysis is a set of tools that organizations are keen to be part of training programs for their employees to be able to analyze the problems that the organization suffers from and solve them once and for all.

However, there are problems associated with this type of training, which is that these tools are not “intelligent” and the results achieved by using these tools depend on the ways of thinking of their users. Appropriate use of these tools will produce correct results and using them incorrectly will lead to wrong results. This is the answer to the question why accidents and problems are repeated continuously

Although RCA methods attempt to identify the root causes of any unwanted event, we have found that there is a huge gap that needs to be filled. That gap is that the ways of “thinking” in which these tools are applied are appropriate for “simple systems” when in reality all systems in which these tools are applied are “complex systems” and require stronger tools to deal with them. This was confirmed by some research done by others that also dealt with this aspect. And this is what happened and is happening even in the largest organizations, as once RCA methods are applied, the same problems recur, accidents recur, waste of resources continues, and the organization’s performance declines.

One of the most powerful tools for solving “complex systems” problems is “systems thinking”.

And because of its knowledge of the methods of systemic thinking, as well as the methods of root cause analysis, we combined them together to produce a developed and intelligent model of “root cause analysis” and we called it
